The report emphasized that Rush’s impatience for progress resulted in numerous safety violations. He frequently ignored advice from engineers, such as when he instructed the crew to use only four bolts to secure the Titan’s heavy dome instead of the required eighteen. This decision led to a catastrophic failure when the bolts sheared off during a subsequent operation.

The US Coast Guard report indicated that financial pressures at OceanGate may have influenced many of Rush’s risky choices. In a bid to cut costs, the Titan was stored outdoors during the harsh Canadian winter, compromising its hull integrity. Ultimately, these decisions culminated in the implosion, described as “catastrophic,” with death occurring almost instantaneously for the occupants.

The sound of the explosion was detected approximately two seconds after the failure, corroborated by communications from the Titan Communications and Tracking Team aboard the Polar Prince. Following the incident, all contact with the Titan was lost, marking the end of the ill-fated expedition.
