LCBO Fined $80K After Worker Critically Injured in Training
UPDATE: The LCBO has just been fined $80,000 after pleading guilty to safety violations following a serious incident that left a worker critically injured during a training session in London, Ontario. This urgent ruling comes from London’s Provincial Offences Court, highlighting significant failures in worker safety protocols.
The incident occurred on June 20, 2022, when employees were being trained to operate a powered pallet lift truck. According to the Ministry of Labour, Immigration, Training and Skills Development, the training session was led by a seasonal worker who lacked the necessary certification to conduct such training. This has raised serious concerns regarding the adequacy of safety measures in place at LCBO facilities.
During the training, the unqualified trainer provided only a minimal overview of the equipment, mistakenly assuming that participants had completed prior theoretical training. This oversight led to a critical mishap when a trainee, unfamiliar with the machine, lost control and crashed into a steel storage rack, sustaining severe injuries.
The ministry’s investigation revealed that the LCBO failed to provide proper information, instruction, and supervision, violating key sections of Ontario’s Occupational Health and Safety Act. This negligence not only endangered employees but also underscores the importance of rigorous safety protocols in the workplace.
Following the court’s decision, a 25 percent victim fine surcharge was also imposed, which contributes to a provincial fund supporting victims of crime, emphasizing the serious implications of workplace safety violations.
